Unilateral Hyperlucent Lung!!


Differential diagnosis:
  • Intrathoracic
    • Pneumothorax (large)
    • Unilateral bullous emphysema
    • Pneumonectomy with compensatory hyperexpansion
    • Airway obstruction (hilar mass, endobronchial lesion, foreign body) with air trapping
    • Swyer-James syndrome
    • Isolated unilateral absence of pulmonary artery (IUAPA)
  • Extrathoracic
    • Unilateral mastectomy
    • Poland's syndrome (congenital absence of unilateral pectoralis muscle)
    • Poor technique / patient positioning
Additional differential diagnosis considerations in the pediatric population:
  • Aspirated foreign body with air trapping
  • Congenital lobar emphysema
